Please complete one application for each child you wish to enroll at The Hope Institute Learning Academy.
If the form is not complete, we will not be able to include your child in the enrollment process. Applications are valid for the 2010-2011 school year only. All complete applications must be received by Friday, March 26th. The lottery will be held in mid April, 2010. Applicants will be contacted with the specific lottery date.
You must submit legible copies of the following forms for your application to be complete.
- Student Birth Certificate or Legal Guardianship Papers
- Parent/Guardian Home Utility Bill, Mortgage Bill or Lease to verify Chicago residency
- Parent/Guardian License or I.D. Card
- Completed New Student Application
